Preparing for your office visit:
Things to bring to the appointment:

Bring your insurance card.

Photo Identification (Driver's license).

If this your first appointment and if you have referral from your referring physician, please bring the referral letter with you. (unless it was faxed to our office).

A list of current prescription or medications and supplements your are currently taking including doses and frequency and bring it with you.

Pertinent information about your medical and surgical history.

Bring if you have any recent x-rays or other pertinent records.

We Value Your Time:
There are certain patient forms that need to be completed prior to seeing the doctor. If you like, these may be filled out before your visit, to save you time. These forms are available on this website, under Documents, or you may call our office and we will mail them to you ahead of your visit.
While emergencies may sometimes occur that cause delays, we strive to honor all appointment times, while giving every patient the time he or she needs to have questions answered fully by the doctor.

Patient Responsibility:
Copays If your insurance company requires that you pay a copay, we are required to collect it at the time of your visit.
Deductibles If your insurance plan requires a deductible,partial payment for your visit is due at the time of the visit, unless your deductible has been met. Our billing company will bill you for any difference, after your insurance company advises them of the amount due, if any.
If your insurance plan has a high deductible, and you would like a payment plan, our office will work with you, at no charge, as a courtesy. Please speak with Rebekah at the front desk.

Referrals If your insurance company requires a referral, please request it from your primary care physician. If your doctor faxes a referral to our office,please ask us to print a copy of the referral for you. Since referrals are patient's responsibility, it is important that you have a copy of your referral so you know how many visits are approved, and the expiration date of the referral.
If you have questions about your insurance benefits, please call your insurance company, using the telephone number shown on the back of your insurance card.
